Immunization Requirements


As of October 1, 2013, Texas state law requires that all new and re-entry students under the age of 22 must provide evidence of a bacterial meningitis vaccination or booster within the past five years and at least ten days prior to the start of school.

All new students, transfer student and re-entering students under the age of 22 must provide one of the following:

  1. Proof of Vaccination – Evidence the student has received the initial vaccination or booster dose during the five years preceding and at least 10 days prior to enrollment.
  • A document bearing the signature or stamp of the physician or his/her designee, or public health personnel (must include the month, day, and year the vaccination and administered)
  • An official immunization record generated from a state or local health authority (must include the month, day and year the vaccination was administered).
  • An official immunization record generated from a state or local health authority (must include the month, day and year the vaccination was administered).
  • An official record received from school officials, including a record from another state (must include the month, day and year the vaccination was administered).

OR

  1. Exemption from Vaccination Requirement
  • Students age 22 and older are exempt.
  • An affidavit or a certificate signed by a physician who is duly registered and licensed to practice medicine in the United States, in which it is stated that, in the physician’s opinion, the vaccination required would be injurious to the health and well-being of the student.
  • An exemption form signed by the student stating that the student declines the vaccination for bacterial meningitis for reasons of conscience, including a religious belief. This form may be obtained at https://corequestjc.dshs.texas.gov/

 

Where can I get the shot?

Check with your private physician or local public health division to see if they administer the shot.  Vaccinations are also available at retail pharmacies such as Walgreens or CVS.
